Scheduling Appointments
Please call in advance to schedule an appointment. If you’re unable to keep your appointment, please call us as far in advance as possible so we may reschedule that time slot. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we’re late due to emergencies or surgery. A $25 fee may apply if 24 hours notice is not given for cancelled appointment.

Your First Appointment
We look forward to your first visit and will help you in any way we can. To help us, we ask that you bring the following to your first appointment:
Any x-rays, MRI scans, CAT scans, EMGs, or other pertinent diagnostic
  test results from any other doctor who has seen you for the same
  problem - if being sent directly to us, please call our office to make
  sure they have arrived before your visit
A list of your current medications
Your insurance card or proof of insurance
A referral from you Primary Care Physician if required by your
  insurance carrier
Any co-payment due at the time of service

You will be asked to fill out a patient background questionnaire, if one is not initially sent to you, so please plan on arriving 15-30 minutes early.

Fees and Payment
We make every effort to decrease the cost of your medical care, and therefore, request payment arrangements for all office services at the time they are rendered, unless prior arrangements have been made. For your convenience, we accept cash, checks, and most credit cards. We will bill your insurance company; however, payment is the patient’s responsibility. If you do not have your co-payment at the time of service, your appointment may be rescheduled.

Insurance
We are providers for most California insurance plans, including HMOs and PPOs. We will help you in every way we can in handling insurance claims and ensuring that you receive the maximum reimbursement possible in accordance with your policy.

HMOs and PPOs: Our office will bill these insurance carriers directly and will accept their reimbursement as payment in full for covered services. You are responsible for any co-payments at the time of service. Be sure to bring a referral from your primary care doctor to each visit, if required by your HMO or PPO. Otherwise, they may not pay for
the services provided.

Commercial Insurance: For your convenience, we will help you to file your commercial insurance claims; however, payment for office visits, medical supplies, and x-rays is due at the time of service.

As a courtesy to our patients, we also accept assignments from Medicare and Medicaid, and handle Worker’s Compensation Personal Injuries.

Prescriptions and Renewals
Please request all prescriptions and authorizations for renewals during normal office hours when our full records are available. One of our office staff will assist you to make sure your prescription is received in a timely manner.

Medical Records Policy
If one of our physicians refers a patient to another doctor, we furnish that doctor with a copy of the patient’s medical records including any pertinent x-ray copies – at no cost to the patient. If medical records need to be obtained for any other reason, a charge of $10.00 plus $0.25 per page will apply. If x-ray copies need to be obtained, a charge of $15.00 per sheet will apply.
